Scalp advancement, also known as hairline lowering surgery, is a technique frequently performed by Dr. Chettasak to correct a high or receding hairline and create a softer, more feminine facial appearance—without the need for hair transplantation.
In this episode, Dr. Chettasak Tulayaphanich (License No. ว.15412) explains the scalp advancement procedure step by step, including the frontal hairline incision, release of the galea from the periosteum to allow free scalp movement, fixation using Endotine or titanium screws, and precise forehead skin excision using a 1:1 ratio.
Closure is performed with a trichophytic (beveled) incision, allowing hair to grow through the scar and creating a natural-looking hairline.
For patients planning Facial Feminization Surgery (FFS), scalp advancement is often combined with forehead bone reduction and brow lift, as these procedures share the same incision and allow comprehensive feminization of the upper face in a single operation.
However, scalp advancement can typically lower the hairline by no more than 4 cm.
In cases of very high hairlines or significant temple recession, hair transplantation may be a more suitable option.

头皮前移术（Scalp Advancement），也称为 发际线降低手术（Hairline Lowering Surgery），是 Dr. Chettasak 经常采用的一种手术技术，用于改善发际线过高或后移的问题，使面部整体看起来更加柔和、女性化，而无需进行植发手术。
在本集中，Dr. Chettasak 详细讲解了头皮前移术的步骤，包括沿前发际线切口、将 腱膜层（galea） 从 骨膜（periosteum） 上分离以释放头皮活动度、使用 Endotine 或钛钉 固定头皮位置，以及按照 1:1 比例 切除额头多余皮肤。

但需要注意的是，头皮前移术通常最多只能将发际线降低 约4厘米。
对于发际线极高或太阳穴明显后退的患者，植发 可能是更合适的选择。
